(1) Vendor needs to provide employee vision benefit services for EyeMed vision including full break down of benefits or plan design, rates for in-network or out of network providers, allowances on frames, lenses, contact lenses, comprehensive eye exam, information on pricing formula for frames, lenses, contact lenses, special options such as blended lenses, special tints or coatings, additional discounts or services offered under your vision plan and any additional information that may be used to review benefit levels or coverage to the government authority located in Knoxville, TN.
(2) All questions must be submitted no later than May 7, 2018.
(3) Contract period will be for one year.

Performance of the work will be Offsite. Vendor needs to carry work in their office location.
