(1) Vendor needs to provide employee dental benefit services for or eligible employees or their dependents including diagnostic or preventive services, brush biopsy, radiographs, periodontal maintenance, emergency palliative treatment, sealants, minor restorative services, crown repair, relines or rebase, implant repair, prosthodontic services and orthodontic services to the government authority located in Knoxville, TN.

(3) Contract period will be for one year.

Performance of the work will be Offsite. Vendor needs to carry work in their office location.
