[A] Budget: Looking for Proposals
[B] Scope of Service:
Vendor needs to provide employee FMLA administration services
- To provide public transit employee FMLA Administration service for the City’s bus transit system
- The Contractor must review existing FMLA policy and advise on needed changes and best practices.
- The Contractor must provide documents to employees to facilitate employee requests for FMLA leave.
- The Contractor must provide a weekly report reflecting status of FMLA requests and leaves.
- The Contractor must determine FMLA eligibility for each claim, notifying both the employee and Human Resources.
- The Contractor must provide required legal notices to the employee regarding their request for FMLA leave for compliance with the federal leave statute.
- The Contractor must obtain and review employee protected health information and advise Human Resources regarding the status of FMLA leave.
- The Contractor must provide dedicated assistance for the employee regarding their FMLA leave and keep Human Resources updated on the status of the employee’s absence.
- The Contractor must review employee return to work requirements and coordinate with Human Resources regarding the return to work protocol.
- The Contractor must monitor intermittent FMLA leave for occurrences that exceed the frequency and/or duration approved on the certification.
- The Contractor must provide leave of absence records for Human Resources confidential files at the close of each FMLA claim.
- The Contractor must provide a method of encryption for electronic transmission of employee health or personal information.
- The Contractor must be able to take calls from employees.
- The Contractor must provide a backup for the person dedicated so that there is no support interruption.

Onshore (USA organization Only)
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office premises.
