[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide video production services to produce high-quality short videos and an engaging one-hour documentary for health education/promotion.
- The videos and documentary will highlight the national epidemic of obesity and cardiovascular disease, the dysfunctional state of primary and preventive care, and state’s innovative population health approach through the state heart health network to address the root causes of the problem
- The videos and documentary will focus on the following:
• The particular burden of cardiovascular disease in the state
• Experiences of patients and stakeholders with getting and providing essential heart health care
• Patient and stakeholder perspectives on the kinds of patient-centered preventive and primary care patients need most
• How state is strengthening primary care to deliver patient-centered population health services, like health coaching and motivational text messaging, proven to empower patient to take charge of their own health and save lives.
(2) All questions must be submitted no later than December 14, 2021.

Onshore (USA Organizations Only)
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
